Adult intensive care is expensive. It is not always possible to predict those who are most likely to benefit from this care. Neither conventional indices of gas exchange impairment, nor relationships between inspired oxygen and arterial blood gases, are of great value in clinical decision making or in predicting patient outcome. Conventional respiratory models, upon which current clinical indices are based, are much too simple to simulate clinical reality, and they reflect steady state physiology rather than the dynamic changes which occur in disease processes. Our aim is to develop, validate and apply a new gas-exchange computer model which will simulate dynamic lung physiology and then be used to (i) predict the optimum patient ventilatory treatment which should be applied; (ii) provide a more accurate diagnosis of those patients developing ARDS; and (iii) provide an earlier detection of lung improvement in these patients. Real-time patient data will be fed into the model in order to achieve these aims on-line.
