In developed countries, most bladder cancers are of the transitional cell carcinoma type, which represent the 6th most common cause of cancer deaths in men. This proposal is one component of a mu lti-disci pi inary and multi-centre collaboration with the over-arching aim of establishing a multi-disciplinary research and training environment focused on clinical, biological and genomic problems in urothelium. The specific diagnostic challenges include a) identification of flat and erythematous lesions as inflammatory, dysplastic or carcinoma in situ; b) determination of grade (degree of differentiation) and stage (extent of invasion) of superficial and invasive TCC; and c) identification of non-invasive lesions with the potential to recur or progress to malignant disease. The specific biological challenges include a) defining the relationship between differentiation and malignant potential, b) determining how urothelial pathologies reflect changes at the molecular level, and c) understanding how changes in genotype can drive malignant progression. The specific medical engineering challenges include a) measurement of structure and function of intact urothelium in-vitro and in-vivo; b) computational modelling to relate measured parameters to underlying structure and function; c) the development of measurement techniques that can be used at a cellular level and/or in-vivo.
