The aim of the proposal is to investigate how to help engineering designers formulate theories about failure in the distributed cognition that takes place during both the design process and the subsequent installation, operation and maintenance of the designed system. Our application is for a one-year grant to employ one research assistant each at Cranfield and Loughborough in collaboration with engineering design organisations in the offshore and onshore sectors (Kvaerner Oil and Gas Ltd, and Eutech Ltd) and a regulatory organisation (HSE). The work has several elements: analysing about 300 investigations into accidents using a distributed cognition framework; building models of each main failure mode and models of the space of all failure modes; developing a system that provides templates and heuristics to help designers manage potential failure modes; and evaluating this system by running trials in the collaborating design organisations. The final stage will involve testing the system with designers from other engineering sectors to assess the generality of the approach.
