Accidents in both onshore and offshore process plant have had disastrous consequences - for loss of human life, loss of revenue and severely degraded environment. An analysis of both the immediate and latent causes of these accidents reveals the central role played by misconceptions: principally designers' misconceptions about the intentions of operators or maintenance staff, and operators' misconceptions about the intentions of designers. The purpose of this feasibility study is to extract this knowledge from about 500 investigative reports and direct it at the primary decision making points in both the design process and operating process (principally the points at which the plant undergoes major changes of state). This will involve a) developing computing mechanisms to represent and record different types of misconception, b) investigating the nature of decision making points in the design and operating processes, c) developing adaptive agenda-generating mechanisms, and d) evaluating the effect of incorporating the mechanisms in the organisational routine.
