Replacement of the hip has become the most frequently performed major surgical operation with between 40,000 to 50,000 operations in the UK alone. With patients living longer and with the operation being performed on younger, more active patients, attention recently been placed on increasing the longevity of hip implants which currently have a mean period of satisfactory function of approximately 12 years. To improve current total hip implant systems, a better understanding of the way in which loads are transmitted in the healthy hip is needed.The applicant proposes to develop patient specific, highly realistic three dimensional numerical models of the femur based on data obtained from different medical imaging modalities (CT, MRI scans). Numerical models based on high resolution CT scans of excised human femurs, both with and without implants, will be developed. Additionally finite element models of a femoral bone analogue will be developed in parallel. These models will be validated by comparing the predicted numerical response to loading with experimental measurements. The numerical models will also be used to explore the influence of complicating effects, such as muscle force, assumed boundary conditions and the presence of a femoral implant on the predicted stress field.
