Using a previously developed mathematical model of the human respiratory system as a starting point, we propose to develop a mathematical model of the respiratory system of artificially ventilated patients on the adult general ICU. Physiological data from 50 patients who are invasively monitored as part of their routine care will be used to review the models of the respiratory system and determine the parameters within the model which require quantification but cannot be measured directly. A further 50 patients studies will be used to validate the predictions based on the model. As part of the validation, patients will be partitioned on demographic and severity of illness and differences in the parameters between groups analyzed in order to determine the homogeneity of the patient group. Differences between the model predictions and the measured values will not only be assessed quantitatively but they will also be assessed qualitatively in terms of their clinical significance. The long-term aim of the research if the development of the respiratory model is successful is to develop a bedside clinical decision support system which will support both prescribing an artificial ventilation regime and proactive monitoring of its efficacy.
