A clinical decision support system will be developed to assist in managing the patient resident on the high dependency environment, focusing on ventilator management of the ICU patient. The approach adopted involves building a physiological model of the relevant cardiovascular, respiratory and associated processes, implemented as a causal probabilistic network (CPN). This time-discrete model incorporates the uncertainty in physiological measurements and predicts variables like respiratory rate, not as a single figure, but as a probability distribution. Secondly by applying penalty scores to variables that are indicators of the 'utility' of a therapy in relation to clinical objectives, a cumulative probability weighted risk can be calculated. These features enable the model to suggest a therapy which minimises the risks of undesired events (eg abnormal tissue gas partial pressures). Tuning parameters to the individual patient the CPN model can be used; (a) to predict outcome in response to variation in ventilator therapy; (b) in reverse mode to assess or diagnose the pathophysiological state of the patient. Retrospective testing will be performed using data from ventilated patients, followed by the first stages of supervised clinical testing.
