The successful treatment of hallux rigidus, hallux varus and rheumatoid arthritis of the MTP joint using a replacement joint is dependent upon the availability of clinically proven prostheses. In common with other joints, it is recognised that the development and evaluation of such prostheses requires and effective joint function simulator that has been validated by clinical evidence. Currently, no such simulator exists. The proposed programme of research will develop a multi-station MTP joint function simulator, which will then be used to evaluate currently used designs of MTP prostheses. The programme will include clinical collaborations to validate the simulator by comparing the in vivo performance of implanted prostheses with that of in vitro simulator tested prostheses. The validated simulator will then be used as a test-bed for the evaluation of new designs of joints, which should lead to the development and Introduction of better MTP prostheses. The testing (five million cycles per test) will allow comparison between designs and concepts of MTP prostheses. A full tribological assessment of the tested prostheses will be undertaken to provide understanding of the wear mechanisms involved. This work will be augmented with an assessment of any wear debris.
