Many cardiovascular investigations require arterial blood pressure (ABP) to be recorded continuously and for this purpose a thin tube, or catheter, is placed inside a large artery. Alternative methods, not requiring the surgical insertion of a catheter, which are thus called 'non-invasive' (NI-ABP), have been developed in the past 20 years, but it is still not clear if these new methods can substitute the invasive procedure in all its applications. One important new area where NI-ABP might not be applicable and give misleading results, is the continuous assessment of regulatory mechanisms that maintain ABP and brain blood flow within narrow physiological limits. Measurements of cerebral blood flow will be performed in 50 patients, using transcranial Doppler ultrasound, simultaneously with both intra-arterial and NI-ABP, during routine cardiac catheterization studies. Comparing the results of the two methods will demonstrate if it is acceptable to use NI-ABP measurements for assessment of regulatory mechanisms that maintain stable levels of ABP and steady blood perfusion to the human brain. Clinical conditions that could benefit from the results of this study are vasovagal syncope, stroke, carotid artery disease, myocardial infarction, and orthostatic hypotension.
